You’re right. Once line is coupled in completely, ball or nipple sticking out of hose is supposed to be depressed to open the flow. Typically there’s a similar ball inside the coupler that’s opened at the same time.
I think the advice to change the tips is USUALLY a Hail Mary attempt that worked once for someone that had a coupler that wouldn’t depress/open the ball. If one hose can’t flow oil, not much is going to happen!
A & B in and out question……you’re correct though there’s no “suction”. There will always be some back pressure on the “return” line. Some systems have more back pressure than others, but there will never be a vacuum.
